Your privacy is very important to me.  The information below is to assure you of my promise to you in line with GDPR:

• The personal information that I keep on you is for the purposes of contacting you and forms part of your therapeutic assessment. This information includes your: 

name, address, email address, phone number, date of birth, gender and ethnicity.

• All information of this nature is kept in digital form and is password protected to ensure that it is kept safe.
• Your data will be kept for the appropriate amount of time after therapy is finished and then disposed of in line with GDPR regulations.
• The only people who have access to this data are myself, Sara Smith, and my clinical supervisor, Simonetta Juniper, if I am incapacitated through serious illness or death.
• Your data will never be shared with anyone else for commercial use or any other purpose.
• You have the right to see the data I hold on you at any time.
